Faculty Feedback Channel
We are committed to open and transparent communication and feedback channels for faculty at the College of Medicine. As a faculty member, you have many places to share your thoughts, including as members of standing committees, in course evaluations, and through human resources.
Use This Form for the Following Types Feedback
- Proposals for process changes
- Concerns about impacts of programmatic changes
- Concerns about the working environment
- Concerns about communication/expectations from a unit/department
Completing the Form
When completing the form you will have the option to remain anonymous. All submissions are reviewed by the Assistant Dean for Faculty Affairs, Ted Chauvin, PhD, and the Associate Dean for Faculty Affairs and Continuing Professional Development, Radha Nandagopal, MD, FAAP. These are kept in confidence and anonymized (unless the submitter opts to be identified) before any further action is taken. Based on the submission’s content, de-identified information will be shared with stakeholders who may include but are not limited to the Offices and Departments within the College of Medicine. For example, ideas for change within the MD program’s LMS may be directed to the Office of Curriculum, which may triage it to MedTech.
